About Our Services

The ethos of our service is to give individuals the ‘Control’ of how/when/where they want service delivered. Our service gives people the flexibility to pay for their care by way of direct payments, self directed budgets or in some cases, payments made by Social Services. In our Supported Living services, the Local Authority concerned is only responsible for payment of the care component of the service. We also provide outreach service for those who are ready to move on to independent living to provide consistency of approach with a gradual withdrawal of our input to instil confidence. Each Service is tailored to the individual needs.

All Tenants of our Supported Living Services have an Assured/Shorthold Tenancy Agreement which allows them access to Housing Benefits if eligible. We are committed to the local community and we conscientiously strive to meet their needs.

Support Provided

What our tenants would expect from our services:

  • Provision of 24 hour care with sleeping- in staff
  • Availability of 1:1 staff support with personal care and daily activities if required
  • Availability of the Care Manager with 24 hour responsibility
  • Support from our internal Health and Social Care professional workers and facilitating access to Primary Care Services
  • A named Personal Assistant for each tenant to support with assessed needs, formulate and implement comprehensive Support Plans; -Continuous monitoring of mental/ physical health, social/ emotional/ psychological needs with a view of identifying progress/regress; put in measures in place to achieve positive outcomes
  • Ongoing risk assessments and counselling sessions
  • Ensuring effective communication with families and professionals by providing regular updated reports to those involved.
  • Professionals provide support in liaising with statutory agencies and in reviewing benefits, applying for the appropriate benefits to maximize tenant’s income and also exploring innovative ways of delivering services such as direct payments/ Self Directed Support
  • To look at employment/ educational opportunities where appropriate
  • Our Interactive Activity Programme will focus on the individual with emphasis on acquiring skills related to budgeting, cooking, employment,
  • socialisation and orientation within the community.
  • Escorts to appointments, college, employment also social and leisure activities if appropriate
  • Modern and comfortable home and opportunity to live in a family environment where all social, cultural and religious needs can be met.
